Cognitive Decline Treatment Program

A 12-Month Physician-Led Intensive to Address the Root Causes of Cognitive Decline

If you or a loved one are noticing changes in memory, focus, or mental clarity, you are not alone—and you are not without options.

The Cognitive Decline Treatment Program (CDTP) is a comprehensive, 12-month, physician-led functional medicine program designed to identify and address the root causes of early cognitive decline, including memory loss, brain fog, and changes in executive function.

This is not a quick fix.
It is not a one-size-fits-all protocol.

It is a deeply personalized medical program for individuals who are ready to take decisive, proactive steps to protect and restore brain health.

A Thoughtful, Science-Based Approach to Cognitive Decline

Cognitive decline is rarely caused by a single factor. In most cases, it reflects the cumulative impact of:

  • Metabolic dysfunction and insulin resistance

  • Chronic inflammation

  • Hormone imbalance

  • Impaired detoxification and toxin exposure

  • Vascular and mitochondrial dysfunction

  • Gut-brain axis disruption

  • Chronic stress and nervous system dysregulation

The Cognitive Decline Treatment Program is designed to address these interconnected systems together, guided by advanced diagnostics, clinical expertise, and ongoing support.

Your care is led by Cynthia Libert, M.D., a board-certified family physician and certified functional medicine practitioner with advanced training in cognitive decline prevention and treatment.

Who This Program Is Designed For

The Cognitive Decline Treatment Program may be a good fit if you:

  • Are experiencing early cognitive decline, mild cognitive impairment (MCI), or persistent brain fog

  • Have noticed changes in memory, word-finding, focus, or mental stamina

  • Have been told “everything looks normal” but know something isn’t right

  • Have cardiometabolic risk factors, inflammation, toxin exposure, or hormonal issues affecting cognition

  • Are motivated, engaged, and willing to commit to meaningful lifestyle and medical interventions

  • Have family or caregiver support if cognitive challenges make follow-through difficult

This program is best suited for individuals with early or mild cognitive changes. For advanced dementia or acute neurological emergencies, we will help guide you to appropriate care.

What Makes This Program Different

✔ Physician-led, not protocol-driven
✔ Deeply personalized, not checklist-based
✔ Root-cause focused, not symptom-suppressing
✔ Integrated medical, lifestyle, and coaching support
✔ Designed for long-term brain resilience, not short-term reassurance

Most participants complete this program and then transition into our Brain Health & Longevity Membership for long-term maintenance and prevention.

What does “Faith-Based” mean in the Re‑Think Aging Community?

Our program is rooted in a Christian understanding of health, wholeness, and human flourishing. We believe that God’s fingerprints are clearly visible in all of creation, especially in the design of our human bodies, and that caring for our bodies is a form of worship and stewardship.

Being faith-based means that we integrate prayer, Scripture, and spiritual reflection into our approach to health and longevity, always in a way that is compassionate, invitational, and never coercive. We honor each person’s unique journey with God.

Are Your Teachings Based in a Specific Christian Tradition?

I (Dr. Libert) am a non-denominational Christian. My faith is centered on Jesus Christ and the historic teachings of the universal Christian church as expressed in the Nicene Creed (see below). Our community is intentionally open to believers from all denominational backgrounds—Protestant, Catholic, Orthodox—as well as those who may not identify with a particular tradition but are open and seeking God.

We hold to the ancient Nicene Creed, which unites Christians across history and denominations.

In the first three centuries after Christ, the church often had to meet in secrecy and live in seclusion due to periods of persecution. During that time, significant theological debates arose—particularly about the true nature and divinity of Jesus Christ.

When Emperor Constantine gained control of the Roman Empire in 312 A.D., he granted Christianity favored status. Soon, he recognized how divided the church was over key doctrines. To promote unity, he called together a council in the city of Nicaea in the year 325 A.D.

From that historic gathering came the Nicene Creed, a clear statement of Christian belief that has endured as a guiding standard for many Christian churches throughout the centuries.

The version shared below is the 1975 ecumenical text produced by the International Consultation on English Texts (ICET) and published in Prayers We Have in Common. When the creed refers to “the holy catholic Church,” it is speaking of the universal body of Christ’s followers, not any particular denomination.

Nicene Creed:

We believe in one God,

the Father, the Almighty

maker of heaven and earth,

of all that is, seen and unseen.

We believe in one Lord, Jesus Christ,

the only Son of God,

eternally begotten of the Father,

God from God, Light from Light,

true God from true God,

begotten, not made,

of one Being with the Father.

Through him all things were made.

For us men and for our salvation

he came down from heaven:

by the power of the Holy Spirit

he became incarnate from the Virgin Mary, and was made man.

For our sake he was crucified under Pontius Pilate;

he suffered death and was buried.

On the third day he rose again

in accordance with the Scriptures;

he ascended into heaven

and is seated at the right hand of the Father.

He will come again in glory to judge the living and the dead,

and his kingdom will have no end.

We believe in the Holy Spirit, the Lord, the giver of Life,

who proceeds from the Father and the Son.

With the Father and the Son he is worshipped and glorified.

He has spoken through the Prophets.

We believe in one holy catholic* and apostolic Church.

We acknowledge one baptism for the forgiveness of sins.

We look for the resurrection of the dead,

and the life of the world to come. Amen.

* The term “catholic church” refers to the universal church (from the Greek katholikos, which usually means “general” or “universal”).

Do I need to be a Christian to join?

No. While the program is faith‑informed and Christ‑centered, we welcome anyone who is curious, open, or seeking. Many of our members share a desire to grow spiritually, emotionally, and physically. You are warmly invited to participate at the level that feels authentic to you.

What are the basics of your faith statement?

Here are a few of our core beliefs that shape the Re‑Think Aging Community, along with Scripture references that ground these truths:

God’s fingerprints are clearly visible in all of creation, especially in our bodies.

“For since the creation of the world God’s invisible qualities—his eternal power and divine nature—have been clearly seen, being understood from what has been made, so that people are without excuse.” (Romans 1:20)

“I praise you because I am fearfully and wonderfully made; your works are wonderful, I know that full well.” (Psalm 139:14)

The body is the temple of the Holy Spirit, and we are called to be good stewards of our health.

“Do you not know that your bodies are temples of the Holy Spirit, who is in you, whom you have received from God? You are not your own; you were bought at a price. Therefore honor God with your bodies.” (1 Corinthians 6:19–20)

We can experience abundant life through our union with Jesus Christ and participation in the life of the Triune God.

“I have come that they may have life, and have it to the full.” (John 10:10)

“Remain in me, as I also remain in you. No branch can bear fruit by itself; it must remain in the vine.” (John 15:4)

Salvation is a free gift from God.

“For it is by grace you have been saved, through faith—and this is not from yourselves, it is the gift of God—not by works, so that no one can boast.” (Ephesians 2:8–9)

God uses all circumstances—our greatest tragedies, losses, and pain—to shape us into the image of His Son.

“And we know that in all things God works for the good of those who love him, who have been called according to his purpose. For those God foreknew he also predestined to be conformed to the image of his Son.” (Romans 8:28–29)

“Consider it pure joy, my brothers and sisters, whenever you face trials of many kinds, because you know that the testing of your faith produces perseverance.” (James 1:2–3)

The Holy Spirit transforms us by the renewing of our minds.

“Do not conform to the pattern of this world, but be transformed by the renewing of your mind. Then you will be able to test and approve what God’s will is—his good, pleasing and perfect will.” (Romans 12:2)

There is a soul rest waiting for us when we walk with and learn from Jesus.

“Come to me, all you who are weary and burdened, and I will give you rest. Take my yoke upon you and learn from me, for I am gentle and humble in heart, and you will find rest for your souls.” (Matthew 11:28–29)

“There remains, then, a Sabbath-rest for the people of God; for anyone who enters God’s rest also rests from their works, just as God did from his.” (Hebrews 4:9–10)

No. While the program is faith‑informed and Christ‑centered, we welcome anyone who is curious, open, or seeking. Many of our members share a desire to grow spiritually, emotionally, and physically. You are warmly invited to participate at the level that feels authentic to you.

What if I have more questions?

Please feel free to reach out. I would be honored to share more about my faith, our community, and how we integrate spiritual practices with evidence-based functional medicine to help you flourish in mind, body, and spirit.

What if I want to add on the TruHealth test in addition to the TruAge test that comes with the program?

Absolutely—you can! Your Re-Think Aging Community membership already includes the TruAge test from TruDiagnostic, which measures your biological age and provides powerful insights into how your lifestyle is influencing your aging process.

If you’d like to take things a step further, you can elect to add on the TruHealth test and pay only the difference in cost. The TruHealth test expands on your TruAge results by giving you a more comprehensive picture of your wellness markers, including inflammation, immune function, and other key healthspan indicators.

We love when members choose to add this—it’s like upgrading from a snapshot to a full panoramic view of your health, empowering you to make even more targeted, effective changes. You can learn more about TruHealth here: TruDiagnostic.

Why You Might Want to Add TruHealth

If your goal is to protect your brain, extend your healthspan, and live with vibrant energy for decades to come, more data can mean more precision in your plan. While TruAge tells you how you’re aging at the cellular level, TruHealth reveals why.

This expanded panel measures additional biomarkers linked to inflammation, immune resilience, and metabolic health—all of which play a pivotal role in cognitive vitality, hormonal balance, and disease prevention. With both tests, you’ll have a clearer map of where you are now and exactly which levers to pull to move toward your best possible future.

It’s like having both the “big picture” and the “fine print” of your health story—so you can take action with confidence and faith-aligned purpose.

Do you offer a discount for my spouse, significant other, or a friend?

Yes! We love seeing people walk this journey together. When you share the experience—whether with your spouse, significant other, or a close friend—you gain built-in accountability, shared learning, and the joy of supporting each other’s growth in both health and faith.

To make that easier, we offer 25% off the second membership when you and your spouse, significant other, or friend join at the same time. That’s a savings of up to $437.50 for the year.

Why do we do this? Because research shows that when lifestyle and mindset changes are made together, they’re more likely to last—and the benefits ripple into every area of life. This discount is our way of investing in your shared journey toward living well, living faithfully, and finishing strong, together.

What if I want to participate in the Re-Think Aging community, but I don’t want to do the TruAge test?

Yes, you can absolutely participate without the TruAge test. While we encourage TruAge biological age testing because it provides valuable insight and a personalized baseline for your healthspan journey, we also understand that some people feel anxious about obtaining this information—or may simply prefer not to include it for personal reasons.

If you choose to skip the TruAge test, the program is still fully available to you. The only difference is the pricing:

  • With TruAge testing: $1,750

  • Without TruAge testing: $1,400

You’ll still receive all the benefits of the program, including expert guidance, coaching, and support for brain vitality and healthy aging. The choice is completely yours, and we’re here to support you either way.

What are the symptoms of cognitive decline?

Cognitive decline refers to a gradual or sometimes sudden reduction in brain function that affects memory, thinking, focus, reasoning, or emotional regulation. Symptoms vary from person to person and often begin subtly.

Common cognitive decline symptoms include:

  • Brain fog or slowed thinking

  • Memory problems, especially short-term memory

  • Difficulty concentrating or multitasking

  • Word-finding problems or slower speech

  • Reduced mental stamina or clarity

  • Changes in mood, including anxiety or irritability

  • Loss of motivation or confidence in thinking ability

From a functional medicine perspective, these symptoms often reflect underlying biological stressors such as inflammation, poor blood sugar regulation, hormonal imbalance, sleep disruption, nutrient deficiencies, toxin exposure, or impaired brain energy production, rather than irreversible brain damage.

Can cognitive decline be reversed?

In many cases, yes, cognitive decline can be reversed or significantly improved, especially when identified early.

Research now shows that mild cognitive impairment and early memory loss are frequently driven by modifiable factors. When the root causes are addressed through personalized care, the brain can regain function due to its lifelong ability to adapt and rewire (neuroplasticity).

A comprehensive approach may include:

  • Reducing inflammation and insulin resistance

  • Supporting mitochondrial and metabolic health

  • Optimizing sleep and circadian rhythm

  • Correcting nutrient deficiencies

  • Reducing toxin burden

  • Supporting stress regulation and mental resilience

Addressing multiple contributors simultaneously leads to the most meaningful improvements.

Can cognitive decline lead to dementia or Alzheimer’s disease?

Cognitive decline can increase the risk of dementia, including Alzheimer’s disease, but progression is not inevitable.

Large population studies show that up to 40–45% of dementia risk is linked to modifiable lifestyle and health factors, including metabolic health, cardiovascular function, sleep quality, hearing, social connection, stress, and toxin exposure.

Early intervention focused on prevention and brain protection can significantly change long-term outcomes.

Can cognitive decline caused by depression be reversed?

Yes. Cognitive decline related to depression is often highly reversible.

Depression-related memory loss and brain fog are commonly driven by:

  • Chronic inflammation

  • Imbalances in stress hormones (cortisol)

  • Neurotransmitter depletion

  • Blood sugar dysregulation

  • Nutritional deficiencies

  • Poor sleep quality

When these biological drivers are corrected, improvements in mood, clarity, memory, and focus often follow. Brain health and mental health are deeply interconnected and must be addressed together.

Can cognitive decline happen suddenly or quickly?

Yes. While neurodegenerative diseases typically develop over years, cognitive decline can sometimes occur rapidly.

Triggers may include:

  • Chronic stress or burnout

  • Infections or post-viral inflammation

  • Mold exposure or environmental toxins

  • Sleep deprivation

  • Medication effects

  • Hormonal shifts (such as perimenopause or thyroid dysfunction)

  • Metabolic instability

Sudden cognitive symptoms should never be dismissed as “normal aging” and deserve thorough evaluation.

Can cognitive decline from alcohol use be reversed?

Often, yes, especially if addressed early.

Alcohol can impair brain function by increasing inflammation, disrupting gut-brain signaling, depleting essential nutrients (especially B vitamins), interfering with sleep, and suppressing mitochondrial function. When alcohol intake is reduced or eliminated and the brain is nutritionally supported, cognitive recovery is frequently possible.

The brain’s ability to heal improves once ongoing toxic stress is removed.

Can cognitive decline cause anxiety?

Yes, and the relationship is bidirectional.

Cognitive decline can trigger anxiety due to awareness of memory problems or fear of loss of control. At the same time, chronic anxiety can worsen cognitive function by elevating stress hormones, impairing sleep, and increasing neuroinflammation.

Supporting nervous system regulation while restoring brain health often leads to improvement in both anxiety and cognition.

Can cognitive decline lead to death?

Cognitive decline itself does not directly cause death. However, advanced dementia increases mortality risk due to complications such as falls, infections, malnutrition, and loss of independence.

This reinforces the importance of early identification, prevention, and treatment, when cognitive changes are most reversible.

How does functional medicine view cognitive decline?

Functional medicine views cognitive decline not as an inevitable consequence of aging, but as a sign that underlying biological systems are out of balance. Changes in memory, focus, or mental clarity are understood as signals reflecting factors such as inflammation, metabolic dysfunction, vascular changes, hormonal shifts, chronic stress, toxin exposure, or nutrient deficiencies.

This perspective emphasizes identifying why cognitive changes are occurring rather than focusing solely on symptom management.

What does a root-cause approach to cognitive decline involve?

A root-cause approach evaluates multiple systems that influence brain health, including:

  • Blood sugar regulation and metabolic health

  • Inflammatory and immune pathways

  • Sleep quality and circadian rhythm

  • Stress physiology and nervous system regulation

  • Nutrient status

  • Environmental exposures

  • Mitochondrial energy production

Addressing these contributing factors together is often more effective than targeting a single pathway in isolation.

Is the brain capable of improving with age?

Yes. The brain maintains the ability to adapt, reorganize, and form new neural connections throughout life. This capacity, known as neuroplasticity, allows cognitive function to improve when underlying stressors are reduced and supportive inputs are restored.

Research shows that brain function can improve at many stages of adulthood when appropriate lifestyle, metabolic, and environmental signals are addressed.

What scientific evidence supports this approach to cognitive decline?

A functional medicine approach to cognitive decline is informed by several well-established areas of scientific research, including:

  • Peer-reviewed studies demonstrating that certain forms of cognitive impairment can be reversible

  • Neuroplasticity research showing lifelong adaptability of the brain

  • Large population studies identifying modifiable risk factors for dementia

  • Research linking inflammation, insulin resistance, stress hormones, toxin exposure, and vascular health to cognitive performance

Together, these findings support early intervention and personalized strategies for cognitive health.

When should someone seek evaluation for cognitive decline or brain fog?

Evaluation should be considered when changes in memory, focus, processing speed, or mental clarity are persistent, worsening, or affecting daily life. Early assessment is especially important when cognitive symptoms are accompanied by mood changes, sleep disruption, metabolic concerns, or a family history of dementia.

Early identification improves the likelihood of addressing reversible factors and slowing or preventing further decline.
